Admissions
The decision of placement of a loved one to a skilled nursing facility can be difficult and overwhelming. Our admissions staff will help ease this process to help make the transition a smooth one for you and your loved one. Gathering the following information prior to admission will be helpful for all parties:
- Family information, including addresses and phone numbers
- Power of Attorney
- Health Care Proxy designation
- Insurance Cards and insurance information
- Patient Review Instrument/Screen- hospitals will complete the PRI/Screen if you had a hospital stay. Home health agencies can complete this screen if you have not had a hospital stay.
- Copies of bank account statements, property information, securities statements, and trusts
- Copies of your Social Security card, Medicare/Medicaid card, or Veterans information
